1. The document shall consist of one or more individual sheets measuring 8.5 inches by 11 inches,
not permanently bound and not a continuous form. Graphic displays accompanying a document
to be recorded that measures up to 11 inches by 17 inches shall be recorded without charging
an additional fee.
2. The document shall be legibly printed in black ink, by hand, type or computer. Signatures and
dates may be in contrasting colors if they will reproduce clearly,
3. The document shall be on white paper of not less than 20 pound weight and shall have a clean margin
of at least one-half inch on the top, the bottom, and each side. Margins may be used for non-essential notations
that will not affect the validity of the document, including but not limited to form number, page numbers and
4. The first page of the document shall contain a blank space, measuring at least 3 inches by 5 inches, from the upper
right hand corner.
5. The document shall not have an attachment stapled or otherwise affixed to any page. Pages may be stapled together.

The name & address of the Grantee on any document transferring title. (55 ILCS 5/3-5026)
The name & address of Grantee for tax billing purposes. (55 ILCS 5/3-5020)
The name & address of the preparer of the document. (55 ILCS 5/3-5022)
A properly completed transfer tax declaration form must accompany any deed or trust document, or a statement of exemption must be listed. (35 ILCS 305/3)
The name & address of the person submitting a plat for recording. (765 ILCS 205/2)
A School District Certificate signed by the owner, notarized, & identifying the School Districts where the properties are located. (765 ILCS 205/1.005)
Plats MUST contain a) statement of location within Corp. Limits (1 1/2 miles) or municipality. b) Owner's name, signature, & address. c) County Clerk's signature & dated within the last 3 months or last tax bill sent. d) signed by all offices. e) Winnebago County Plat Officer's signature must be within 90 days of date signed. f) four signed copies.
Plat MUST contain: a) Surveyors signature, address, date & seal. b) boundaries of parcel. c) view of building on parcel. d) elevation (side) of unit/s. e) floor plan of each unit & identifying number. f) School District statement with owner's signature, notarized & declaring the school district. g) Four signed copies.
Declaration: Declaration & plat must be recorded together. Declaration MUST contain: a) Legal description of this condo parcel. b) Plat of the condo. c) the % of ownership. d) a legal of the additional land for add-on condo. e) Name of the city & county where it is located. f) Description of the common elements.
A return to or mail to address is required for all documents
being submitted for recording.
